You can use the Synchronizer to integrate both the applications. However you will have to develop an adaptor and deploy it in the synchronizer server. Once the adaptor is deployed, you can create a link and configure it to synchronize bidirectional or unidirectional even on field level.
Note that the Synchronizer can only handle Requirement, Folders and defect entity. If you are planning to synchronize Test cases then synchronizer can not handle it.
