Best way to create defect and Re-test

Couple questions please:

1. Is the best way to create defect is via the 'New Defect' option when Run test and failed at certain 'step'?

2. OR go to 'Defect' and 'create NEW'?  But how to link the failed test scripts to this new Defect?

Also when a defect is created, either via 1) or 2), when it is fixed, can Tester just go to Defect and run tests (RE TEST) from there? currently I cannot find a way to take me directly to the failed test case/step via the Defect.

HELP please.
