The reference I provided doesn't include any sample code.
Refer to the Administrator's Guide > Project Customization > Generating Workflow Scripts > Customizing Defects Module Dialog Boxes
In that section is an image of the Defect Details Field Customization dialog.
Step 7 (in my copy of the document) says:
"You can set the Add Defect and Defect Details dialog boxes to include one or more input pages. By default, all fields are displayed on one page. Use the up and down arrows to move fields to the appropriate page."
You should be able to simple drag the fields from page 1 to page 2..n, according to this documentation.
