How to create a create a QC report containing Folder structure, Test set and test case details

Greetings team,

In Test Lab, I have 4 level folder structure containing a test set[Pls find attacched screen shot]. Need your help to generate a report with following details.

1. Complete Folder structure
2.  Test Set name
3. Name of each test case within test set
4.  Status of each test case
