I've seen these red Xs in the Dashboard and Test Lab modules, when there is something wrong with the data stored for that record. In one case, I traced it to a field that is stored as html and it was missing a closing tag. In another case, it was an invalid or corrupted characters in one of the fields. I deleted the data in those fields and the X went away.
