I just did migration from QC 9.0 to ALM 11.5 on a test environment.
In your case, there is no direct path to upgrade v9.2 to 11.5.
I have few below important suggestions for you.
1) Repository over database
Repository over database is supported from QC v10.0 and above.
2) Upgrade assessment tool
Install and run the upgrade assessment tool to verify schema and other issues..This utility also tells you if you are repository over database. (Download the assessment tool for v9.2)
HP has upgradation preparation guide published and I think this is the best one I have seen so far...You have solution for most of the known issues and ensure you follow the process as mentioned in this document.
3) if so, you need to be on atleast QC 9.2 Patch 12.
'Read Me' of patch 12 helps you with the process to move repository to file systems.
4) Mixed table ownership: Run the queries mentioned in above document and it should solve any 'td' related issues.
I suggest you try this on a test environment for a smaller project or demo project and document the steps yourself :)
