You can create a vba (in excel) or vbs script which will connect to the first project (project user creates the request) and reads the requests of status new.. and in the same script you can connect to other projects (I assume this will be specified in the request) as required and perform the add User method.
As Trudy said, you can then set this vbs/vba to run at specific intervel using Windoes Task Scheduler..
Couple of things to Note
1. User who connects to the project using OTA should have enough rights to add user to the project
2. You should such a way to determine if user should be added to Site Admin or just to project. The first requires Site Admin access.
